Dear Convenors,

In order to streamline the registration process, HKHAMS has decided that Re-registration Form is no longer required to be submitted for ALL local player above 18 year-old without any change of team and shirt number. Club officials only have to sign a one-time declaration form that will be valid for whole season. Once the re-registration applications are submitted by the club online, they will be deemed as final submission and HKHA will approve accordingly. Missing information or inappropriate size of player’s photo will not be accepted. For the standard procedure, please refer to the attached file “MS - New Arrangement of Re-Registration Process”.

Please complete and sign the declaration form and submitted to jowiewong@hockey.org.hk by 12:00pm on 24 Aug 2018 (Fri).
