Parent Consent Form - Special request - Player Re-registration

Dear Convenors,

P&D Section will organize the Youth League (7-a-side) starting from 2 November 2024 to 29 March 2025. This year we shall have 3 different divisions: mixed teams , boys’ teams and girls’ teams.

If your club is interested in joining the league, please complete the Club Registration via: (https://forms.gle/j4wRae8L25bPrtyWA) on or before 15:00, 2 October 2024 (Wednesday). We will preliminarily collect the team component details to decide the playing format in the first stage.

Payment of 1) Club Affiliation Fee: $200 per club

2) Team Registration Fee: Maximum $900 per team (Depends on the no. of team entry, maybe less than $900.)

Player registration:

Player registration deadline: on or before 13 October 2024 (Sunday).

Age range: born on 1 January 2008 - 31 December 2012 (Must reach 12yrs old on registration date)

Minimum registered players in a team: 9 players

New Player Registration Form (http://www.hockey.org.hk/YouthRegistration.asp) / Online Player Re-registration must also be submitted through the website registration system. For any special request e.g. shirt # change / player transfer from other clubs, please complete the respective form attached and send to andykwok@hockey.org.hk.

Registration submitted after deadline will be processed in next batch. Please ensure sufficient players registered before the deadline to join the league.
